This form captures one matter type at a time. Please submit it once for each Notarial Act type you handle. Allow about 10 to 15 minutes per matter type — Notary work is shorter than other practice areas, but the structure is the same.
If a Notarial matter type runs differently in scenarios that change the scope, fees, or documents, please submit a separate form for each variation. For example, an apostille for a single document and an apostille for a bundled set of overseas-bound documents may have different scope, fees, and documents — submit one form for each. Use the Variation field on the next page to label what's different about this submission.
Input as much as you can. If you're not sure about something, type PLACEHOLDER in that spot rather than leaving it out or guessing.
