If a matter type runs differently in scenarios that change the scope, fees, or documents, please submit a separate form for each variation.

For example, a Business Sale with finance and a Business Sale paid in cash may have different scope, fees, and documents — submit one form for each. Use the Variation field on the next page to label what's different about this submission.

Input as much as you can. If you're not sure about something, type PLACEHOLDER in that spot rather than leaving it out or guessing.

Example for Stages:
